Below are draft of forms (pdf format) designed to support information gathering about students, or to be used by school systems to assess implementation of strategies to support students from diverse backgrounds. Included are a (1) sociocultural checklist, designed to collect background factors about a student and/or their family (2) a checklist to gather information about protective and positive factors associated with a student and their family and (3) a checklist to reflect on aspects of the assessment process, focusing on means to reduce bias.
